I give ____________________________ permission to sign in/out of East Haddam Parks & Recreation Middle School Day Camp at 8:30 a.m. and 3:00 p.m. on the following date(s): __________________________________________________. I understand and accept all responsibility for my child before he/she has signed in to camp no earlier than 8:30 a.m. and after he/she has signed out of camp at 3:00 p.m. 

I agree to hold harmless the Town of East Haddam and East Haddam Parks & Recreation for anything that may occur with my child before 8:30 a.m. and after 3:00 p.m. I understand that my child will not be allowed to leave camp until 3:00 p.m., unless released to a parent or guardian.
